For Adelante Healthcare’s 2024 Annual Report, the goal was to move beyond a standard review of financial sustainability and operational data, and instead tell a deeper, more impactful story of what it means to be a Federally Qualified Health Center. In addition to the typical reporting on patient demographics, employee diversity, and financials, this year’s report highlighted key organizational achievements and patient stories that reflect Adelante’s Culture of Care. These real-life narratives showcased how Adelante’s services are changing lives and reinforced the human impact behind the data.

The report’s design was led by the organization’s Cultural Transformation initiative which was an organizational shift that defined much of Adelante’s efforts in 2024. This report celebrates Adelante’s core brand identity, strengthening the storytelling and bring consistency to the visual experience. With the support of the Executive team, and close collaboration with the data team and senior leadership, we were able to present more robust and transparent data than ever before. The result was a powerful, visually cohesive piece that not only informed but inspired.

Finding Meaning in the Moment - Ana's Story

Ana was born with limited vision and one day, she lost the rest of it in a blink of an eye. In her darkest moment, she nearly gave up, but her faith pushed her to keep going. She found support at the Arizona Center for the Blind and Visually Impaired and later at Adelante Healthcare where she felt welcomed and cared for as a whole. Toda, Ana is more determined than ever to continue her story and live her life to the fullest.

Surviving the Attack of CIDP - Aishia's Story

Aishia was a successful healthcare executive and busy mom when she was diagnosed with CIDP, a rare autoimmune disorder that left her unable to walk or talk. Her life changed overnight, forcing her to relearn basic skills and navigate an entirely new reality. She sought out care from her neighborhood clinic, which happened to be one of Adelante Healthcare’s comprehensive sites, where she was able to feel supported. From diagnosis, to nutrition, behavioral health and even facilitating her referrals, Adelante has walked along side Aishia as she’s made major strides in her recovery.

Reclaiming Life From Depression - Kayleene's Story

At 16, Kayleene had hit one of her lowest moments of her life. Battling depression and struggling to express herself, she found support from her mother, who encouraged her to seek help at Adelante Healthcare. Here she connected with a therapist who offered her the support she needed to start healing. Over time, she’s regained her confidence, earned her GED, and reconnected with the bubbly, bright person she once was.
